Patient Care Coordinator

The Patient Care Coordinator is responsible in ensuring efficient and effective patient care delivery within our healthcare facility. This role will also be responsible for managing prescription requests, addressing medical queries from patients and healthcare providers, and coordinating various administrative tasks to facilitate seamless patient care processes.


  • Receive and process prescription requests from patients and healthcare providers.
  • Ensure accurate and timely processing of prescription refills and authorizations.
  • Complete all prescriptions and prior authorization for all medication.
  • Assist in Telemedicine and Virtual Check in.
  • Coordinate with pharmacy services as needed to facilitate prescription fulfillment.
  • Respond promptly and courteously to medical queries from patients and healthcare professionals.
  • Provide accurate information regarding medical procedures, appointments, and healthcare services.
  • Collaborate with medical staff to address patient concerns and inquiries effectively.
  • Coordinate patient referrals to specialists and other healthcare providers.
  • Maintain electronic medical records (EMR) and ensure data integrity and confidentiality.
  • Utilize ophthalmic knowledge to assist patients with eye care-related inquiries and services.
  • Verify patient insurance coverage and assist patients with insurance-related inquiries and paperwork.


  • Experience with patient care coordination, medical records management, or administrative duties is beneficial.
  • Proficiency in medical terminology and familiarity with electronic medical records (EMR) systems.
  • Strong attention to detail and accuracy in handling prescription processing and administrative tasks.
  • Clear and coherent in both written and verbal communication skills with the ability to interact empathetically with patients, colleagues, and healthcare professionals.
  • Ability to multitask efficiently across various platforms and prioritize tasks to meet patient care and administrative needs.
  • Ability to work both independently and collaboratively in a fast-paced healthcare environment.

Screening Criteria

  • Bachelor’s degree in a related field.
  • At least two (2) years of experience in a medical office or healthcare setting.
  • Must have a stable employment history.
